Privacy policy

This Privacy Policy applies to the information collected in connection with the Farmacy Ohio program (“Program”) and stored on the Farmacy Connect platform (the “Platform”). When you sign up for the Program, certain information may be collected, stored, and shared by the Ohio Association of Foodbanks, Mid-Ohio Food Collective (MOFC), your local food bank, your local agency partner and the healthcare provider that referred you (“Farmacy Ohio Partners”). The Farmacy Ohio Partners will share your information through the Platform to provide food and other services to you in accordance with this Privacy Policy.

Information Collected by the Farmacy Ohio Partners in Connection with the Program

When you participate in the Program, the following information may be collected about you:

• Contact Information: The Farmacy Ohio Partners may collect your contact information, such as your first and last name, email address, phone number, address, state, ZIP/Postal code, and city.
• Communication Records: If you communicate with any of the Farmacy Ohio Partners about the Program, the Farmacy Ohio Partners may keep records of those conversations.
• Demographic Information: The Farmacy Ohio Partners may collect demographic details about Program participants like birthdate, gender, and other similar details.
• Household Information: Other information may be collected about your household if you utilize the Program to pick up food for your household.
• Healthcare Provider/Insurance: The name of your current healthcare provider and insurer may be stored on the Platform and shared with other Farmacy Ohio Partners.

How Your Information is Collected

Your information may be collected from the following sources:

• You: The Farmacy Ohio Partners collect information from you whenever you visit, call, or contact one of the Farmacy Ohio Partners or otherwise provide information in connection with the Program.
• Food Bank Agency Partners: Your information may be collected by your local agency partner each time you make a visit or otherwise communicate with such partner about the Program.
• Third Party Platform: Your information may be pulled from a separate platform which is governed by a separate Privacy Policy. Please review such third-party Privacy Policy with respect to how your information may be handled.
• Healthcare Providers/Insurer: Your healthcare provider or insurer may collect your information in connection with the Program in order to verify eligibility for and use of the Program and to connect you with the resources you need through the Program.

Use of Your Information

The Farmacy Ohio Partners may use your information for the following purposes:

• Fulfillment: To provide food and other services and to respond to your communications, inquiries, and feedback.
• Payment for Services: The Farmacy Ohio Partners may receive funding in connection with the services they provide to you. To validate this funding, the Farmacy Ohio Partners may need to share information about your use of their services.
• Communications: Communicating with you, including by responding to your requests, sending you emails, texts, or other messages, asking for feedback through surveys or other messages, providing related information, or making recommendations in connection with the Program.
• Research: The Farmacy Ohio Partners may use information pertaining to participants in the Program and Program outcomes for research and development purposes.
• Security: To protect the Farmacy Ohio Partners and participants in the Program from fraud, security threats, and other illegal or harmful activity. The Farmacy Ohio Partners may disclose specific individual information if necessary to: (i) investigate any suspected misuse of the Program; (ii) protect and defend the rights and property of the Farmacy Ohio Partners, or the interests of the Farmacy Ohio Partners’ third party service providers; (iii) prevent fraud; or (iv) respond to security breaches.
• Legal Obligations: To comply with legal and regulatory requirements and in responding to requests from courts or other government bodies.

How the Farmacy Ohio Partners Share Your Information in Connection with the Program

For purposes of the Program, the Farmacy Ohio Partners may share your information with:

• Farmacy Ohio Partners: Your information may be shared with other Farmacy Ohio Partners in order to facilitate your participation in the Program. Please Note: The Farmacy Ohio Partners will also separately collect and use your information in accordance with their own privacy policies.
• Service Providers: The Farmacy Ohio Partners partner with third parties to assist with the operation of the Platform and the Program, provide services to you, and communicate with you. The Farmacy Ohio Partners may share with them any types of information described in Information Collected by the Farmacy Ohio Partners in Connection with the Program. The Farmacy Ohio Partners may also receive information collected by these third parties and combine it with information the Farmacy Ohio Partners have collected.
• Other Third Parties: The Farmacy Ohio Partners will disclose information about you, including to government bodies or law enforcement agencies, when such sharing is necessary for compliance with the law or to protect the Farmacy Ohio Partners, the Platform, the Program, or the public.

Your Privacy Choices

If you no longer wish to participate in the Program, please contact farmacyconnect@mofc.org. Your information will be removed from the Platform. Please note that even if you communicate that you no longer wish to participate in the Program, your information will still be separately collected and used by the Farmacy Ohio Partners in accordance with their own privacy policies if they provide other services to you.

Changes to this Statement

This Privacy Policy may be updated from time to time and will be emailed annually to all valid Program participant emails on file with the Farmacy Ohio Partners. If you do not have an email on file, the current version of this Privacy Policy will be available at farmacyohio.org.
