Farmacy Ohio links income eligible and enrolled patients to the fresh produce needed to manage and improve health by providing weekly access to fresh produce through Farmacy Ohio participating food pantries. The program empowers healthcare providers with the data and information needed to support patient health, including regular reports on program utilization.
Helping families with
food-based health care
Though many patients do have a long term health condition, many healthcare providers screen and refer 100% of food insecure patients to the Farmacy Ohio program, regardless of if they have a health condition.
Farmacy Ohio is a partnership between health care providers, insurers, the Ohio Association of Foodbanks and patients, with the goal of supporting people with nutritious food to prepare at home. We do not give patients any form of medicine through Farmacy Ohio. We give weekly access to fresh produce so that healthy food access is not a barrier to improving health.

Farmacy Ohio connects patients to fresh produce that will improve health outcomes of all members of the household. The goal is to establish a systematic way to screen for and address food insecurity through an electronic, closed-loop referral system, which connects patients to fresh produce from a food pantry near their home.
Is this an evidence-based approach?
A pilot program that Farmacy Ohio was a part of in 2015 working with individuals with diabetes, found that by giving patients diabetes-appropriate food, blood sugar monitoring, primary care referral, and self-management support, there was a significant improvement in mean HbA1c from baseline (8.11 percent) to follow-up (7.96 percent).
There were also significant improvements in fruit and vegetable intake, self-efficacy, diabetes distress, medication nonadherence, and trade-offs between buying food or medicine.
