How to join

Step 1

Talk to your doctor about joining Farmacy Ohio.

If your doctor participates in the program and you are eligible, your doctor will give you a special Farmacy Ohio Card.

Your Farmacy Ohio Card

If you’re picking up produce for the rest of your household, please let the agency know your household size. Your household may have multiple Farmacy cards. Keep in mind produce may only be provided once per week for the entire household.

Photo Identification Card

Please bring a valid picture ID. This could be a driver’s license, ID card, or passport. Picking up food for someone else? Learn about Proxy Shopping.

A document that shows you live in Ohio (also called ‘proof of residency’)

This document should have your name and current address, dated within the last 90 days. This could be a utility bill, a lease agreement, a medical bill, or Ohio Department of Job and Family Services Announcement.

Frequently asked questions

There are a number of participating health providers who can make a referral for the Farmacy Ohio program. If your provider is a participant, ask if you qualify for Farmacy Ohio at your next office visit, and start using your Farmacy Ohio benefits to get fresh food right away.

Farmacy Ohio pantries allow enrolled members to pick up produce items weekly. Those pantries may also allow you to pick up other types of food based on their normal policies and procedures. It’s important to note that the Farmacy card does not grant double or extra food compared to other shoppers.

Picking up food is free as long as your household income is below 200% of the Federal Poverty Guideline.

Yes. You can send a proxy to get your food for you and your household. Just click here and fill out the form, or write a letter that includes the information on the form and have the person designated to pick up your produce bring it with them when they come to the agency site.

No. Farmacy Ohio enrollees must follow the same intake process as other pantry visitors, including pre-registration requirements and/or waiting for service.

You should bring your Farmacy Ohio ID card, which you will receive from the foodbank through your preferred method of contact - email, text, or mail, and proof of residency.

There are trained, helpful staff at each pantry to help you with your first visit. Each pantry is just a little different, so calling ahead may be helpful if you have specific questions. Please bring your Farmacy Ohio ID card for faster check-in. Bringing your photo ID and proof of residence may be helpful, but is not required.

It’s not. This is the same program. If we enroll you are enrolled in MOFC’s Farmacy program, your healthcare provider will receive data about your pantry use.
